Simply download and print our "Fax Your Order" form, and fax it to us. Upon receipt, we will process your order quickly. It is in .pdf format for easy use. 

Its really simple to do. Here's how:

• Fax the order form to us at the number listed on the form.
• Upon receipt we will process and ship your order. Also, upon shipping, we will send you an email with your shipment details.
• If you select to have us call you for payment information, please indicate that on the form along with a phone number and time to call.
